Summer Session Registration

The registration form for the summer session session is now available. A completed and signed registration must be received with payment for every player by April 30th, 2010. Any player not doing so by will not be allowed to play until payment is received.

Please print out the form, fill in your child's information, and fill out and sign the waiver. Include your coach's last name and the age division (e.g.; U12 boys) where indicated. Mail in the form to the address listed. Do not give it to your coach. Click here to download the form.
