2011 Summer Registration

Registration payments for the 2011 summer session are now due. Please print the registration form, and mail it in with your payment. Fill in your child's information, and fill out and sign the waiver. Include your coach's last name and the age division (e.g.; U12 boys) where indicated. Mail in the form to the address listed. Do not give it to your coach.

A completed and signed form must be received with payment for every player in U10 - U14 by April 22, and for U15 - U19 by May 20. Any player not doing so by will not be allowed to play until it is received. Click <here> to access the form.
